A guide to completing your Inspire Wellbeing referral form

Your first step to support – made simple and stress-free

At Inspire Wellbeing Services, we know that taking the first step to seek support can feel daunting. Whether you are looking for help for yourself, your child, or a loved one, we want to make the process as smooth and stress-free as possible. 

Completing our referral form is the first step to accessing our services.  We have created this guide to help you understand why we ask certain questions, how this information helps us, and what happens next. 

Why do we ask for this information?

Each question on the form is designed to help us: 

  • Understand your needs – This ensures we allocate you the right support. 
  • Prioritise your safety and wellbeing – Some information helps us assess if there are any safeguarding concerns we can support you with. 
  • Reduce delays – A fully completed form allows us to process your referral quickly and efficiently. 

Next, we go through the form step by step so you know what to expect. 

Step 1: Contact details

Helping us get in touch with you easily and securely.

We start by asking for: 

Your personal information

This allows us to create your record and make contact with you.

Emergency contact details

This is only used if we are ever concerned about your safety.

Home/ postal address

This helps us determine location-based support.

How you heard about us

Knowing how people find us helps improve our communication.

Why is this important?
We need basic contact details to communicate with you, send important information, and ensure you feel supported from the start. 

Tip: If you prefer certain contact methods (e.g., email over phone calls), let us know. 

Step 2: Your background information

Helping us tailor support to your individual needs.

Here, we ask about: 

  • Any neurodevelopmental or learning difficulties – This helps us understand any additional needs. 
  • Any disabilities or medical conditions – If you have specific health needs, we want to ensure our services are accessible and appropriate for you. 
  • Your relationship status and household members – Knowing who you live with can help us understand your support system. 

Why is this important?
Mental health is influenced by various factors, including health, learning needs, and family dynamics. This section helps us gain a better picture of your situation. 

Tip: If you are unsure how to answer, just provide what feels relevant—we will ask for more information when needed during the initial telephone call.  A referral to us also includes an assessment process, so you will be asked for further details in your first session. 

Step 3: Your support history 

Understanding what support you have had before – and what has worked for you.

We ask about: 

  • Any recent referrals to other services – This helps us to assess if we are the most appropriate service to support you. 
  • Whether you have received support from Inspire Wellbeing Services or Kids Inspire before – If you have received support from us in the past, we can look into your previous support and speed up parts of the referral process. 
  • Any waiting lists you are on for mental health support – It is not effective for more than one service to be involved at the same time. This is because it is likely that the therapy plan offered to you will conflict with the plan offered by another service.  It can also be confusing as the approaches and ways therapists work will be differ from therapist to therapist and from service to service.   If you are on a waiting list for support elsewhere, we may ask for further information to assess whether it is appropriate for us to support you at the current time.  

Why is this important?
By understanding your past experiences with therapy or mental health services, we can avoid repeating unhelpful approaches and focus on what works best for you. 

Tip: If you have had therapy before and found certain techniques helpful (or not helpful), please let us know.  

Step 4: Your symptoms and concerns

Helping us understand what is bringing you to therapy.

Here, we ask you to tick any symptoms or concerns that apply to you. 

We also ask about background experiences that might be relevant for us when we are considering the best approach for your support. 

Why is this important?
This helps us understand your experience in a clear and structured way. It also allows us to identify underlying factors that may be affecting your mental health. 

Tip: If you can’t see your concern listed, use the “Tell us more” section to describe what is going on in your own words. 

Step 5: What you hope to achieve from support

Focusing on what matters most to you.

Here, we ask: 

  • What has been most challenging for you? 
  • What changes or improvements you would like to see? 

Why is this important?
Therapy is effective when it is goal oriented. Even if you are unsure about your goals, this section helps guide our work together. 

Tip: There are no wrong answers! Whether you want to feel calmer, improve relationships, or simply ”figure things out”, all goals are valid. 

Step 6: Your availability

Ensuring therapy fits into your schedule.

We ask: 

  • When are you available for sessions? 
  • Would you prefer online or face-to-face sessions? 

Why is this important?
The aim is to find a weekly time that will work for you for the duration of therapy, so that you can attend consistently and get the most out of your support. 

Tip: If your availability is limited, please let us know. We will do our best to accommodate but it may take us slightly longer to allocate a therapist for you. 

Step 7: Giving your consent

Understanding your rights and privacy.

To work with us, we need your consent to:

  • Store your information securely 
  • Share information with relevant professionals (only if necessary and requested by you as our client – providing there is not a safeguarding concern) 
  • Record sessions (for training or quality improvement, if agreed) 

You have control over your information. If there is anything you do not want shared, you can specify this on the form. 

Why is this important?
Your privacy is a priority. We use your information only to support your wellbeing and ensure you receive the right support. 

If you are a child or young person, and you are being seen at school, we do not share your information with your teachers or other members of staff.  However, if we are concerned about you, we have a duty to contact the member of staff in your school responsible for safeguarding. If that decision needs to be taken, we will discuss with you beforehand, and will also contact the Inspire Wellbeing Service Safeguarding Team so that we can all work together to ensure your safety.   

Tip: If you have any concerns about confidentiality, feel free to ask—we are happy to discuss. 

What happens next?

1 We review your referral 

Our team will carefully review your form to determine if we are the most appropriate service to support you. 

2 We contact you 

You will hear from us within one working day to discuss your next steps. At this stage we will either: 

  • Signpost you to another more appropriate service 

3 Your therapy journey begins 

Once everything is in place, we will allocate you a therapist for your initial assessment session.
